If you are living with HIV/AIDS, you can count on AIDS Vancouver’s Case Management program. We work to help you achieve stable and sustained health, and a better quality of life.
The program provides information, support, and care coordination for you and your family. AIDS Vancouver’s Case Managers work to meet your unique needs through thorough assessment and care planning. We link you to services and resources. We can also advocate for you to address barriers to your health. Our immediate focus is on food, shelter, income, and emotional support, but we may also work with you over the long-term in more complicated areas such as:

Call 604-893-2270 (9:00am - 5:00pm PST Monday to Friday). You can leave a confidential message, and a Case Manager will return your call as soon as possible.
Visit Us
You can drop in to our Access Office at 1107 Seymour St. between the hours of 11:00am and 4:00pm, Monday through Friday.

Home/Hospital Visits
If you are in a local Vancouver hospital, your hospital social worker can arrange for an AIDS Vancouver Case Manager to visit you. If you are ill at home, please call 604-893-2270 to arrange for a home visit.
